Comments:
A subsidy health and safety inspection was completed on August 20, 2019 from approximately 2:00 p.m. to 4:15 p.m. to review supplemental health and safety requirements. Thirty children were in care and five staff were on premises during the inspection. Staff and children were observed in the indoor space engaged in activities to include snack time, center play, and departure from the program. Interviews and interactions were conducted and medications were reviewed. Five children?s records and five staff records were reviewed. The Director was available for the inspection and present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-520-B-11
Description: Based on a review of records and interview, the licensee failed to maintain a written allergy care plan for a child with a diagnosed food allergy for two of eight children.
Evidence: 1. Child #3 and Child #6's records indicated diagnosed food allergies. The records lacked written allergy care plans as required for children with diagnosed food allergies. 2. Administrator #1 acknowledged the center does not have these written allergy plans.

Plan of Correction: The Director will obtain these forms from the parents.

Standard #: 22VAC40-665-540-A
Description: Based on a review of staff records and interview, the licensee failed to have staff evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB), submitted at the time of employment, prior to coming into contact with children, and completed within the last 30 calendar days of the date of employment for two of five staff.
Evidence: 1. Staff #3's record, hired on August 15, 2019, contained a tuberculosis screening/test dated April 12, 2018 and therefore not within the required time frame. Staff #5's record, hired on August 12, 2019, contained no tuberculosis screening/test documentation and therefore not within the required time frame. 2. Administrator #1 acknowledged that these tuberculosis screenings/test were not completed at the time of employment.

Plan of Correction: The Director will have these staff obtain tuberculosis screenings/tests and submit documentation.

Standard #: 22VAC40-665-580-B
Description: Based on review of staff records and interview, the licensee failed to ensure that within the first 90 days of employment all staff who work directly with children shall complete Virginia Preservice Training for Child Care Staff for two of five staff.
Evidence: 1. Staff #1 and Staff #2's records, both hired on May 6, 2019, lacked documentation of Virginia Preservice Training for Child Care Staff. 2. Administrator #1 acknowledged these staff have not completed this training within 90 days of employment.

Plan of Correction: The Director will have these staff complete this training.

Standard #: 22VAC40-665-580-E-1
Description: Based on a review of staff records and interview, the licensee failed to ensure that within the first 90 days of employment all staff who work directly with children shall obtain current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care for one of five staff.
Evidence: 1. Staff #2's record, hired on May 6, 2019, lacked documentation that CPR has been completed. 2. Administrator #1 acknowledged that this staff has not competed this within the required time frame.

Plan of Correction: Per the Director, the staff is scheduled to take the training on August 30, 2019.

Standard #: 22VAC40-665-580-E-2
Description: Based on a review of staff records, the licensee failed to ensure that within the first 90 days of employment all staff who work directly with children shall obtain current certification in first aid for one of five staff.
Evidence: 1. Staff #2's record, hired on May 6, 2019, lacked documentation that FA has been completed. 2. Administrator #1 acknowledged that this staff has not completed this within the required time frame.

Plan of Correction: Per the Director, the staff is scheduled to take the training on August 30, 2019.
