Comments:
An unannounced inspection was conducted on July 11, 2019 in between the times of 10:40am to 12:10pm in response to a self reported incident that was received in the licensing office on July 2, 2019 regarding noncompliance in the areas of staffing and supervision. The licensing inspector toured the facility and reviewed supervision, physical plant, and administration. Based on the information gathered, the self reported incident was found valid. There were violations cited in the areas of staffing and supervision and operational responsibilities. An exit meeting was conducted with the facility director prior to the end of the inspection.

Violations:
Standard #: 22VAC40-185-40-E
Description: Based on record review, the operational responsibilities of the licensee did not ensure that the center's activities were maintained in compliance with these standards, and the center's own policies & procedures that are required by these standards. Evidence: 1. On July 1, 2019 Staff #1 did not follow the centers policy #4.2.4 (child safety and supervision) which states " Using name to face sheets will assist in maintaining accurate attendance records of the children in care. Perform name to face attendance at the following times: Going to and from the playground and transitioning from one location to another within each classroom group" This document outlines the policies related to care and protection and sight sound and supervision of children. 2. During interviews with Staff #1 it was confirmed that Staff #1 failed to perform a name to face child count prior to leaving the playground to enter back into facility. 3. During the interview and record review it was determined that Staff #1 signed child# 1 out on the name to face sheet at 5:35pm documenting that the child was no longer present at the program, while she was actually still in the care of the center.

Plan of Correction: Staff Member #1 was terminated on 7/23/19 all center staff were retrained on policies that included: Transitions, Name to Face Sheets and Supervision. Effective immediately, managers over see each transition to verify ans support all transitions are in compliance in and out of the building.

Standard #: 22VAC40-185-340-A
Description: Based on video observation and interview, the provider did not ensure that staff while supervising children were ensuring the care and protection of children. Evidence 1. On July 1, 2019 a four year old child was left alone and unsupervised on the outdoor playground for about 20 minutes, shortly before the time of 6:00pm in the evening. Child #1 that was left on the playground sat on the ground and walked around on the playground and appeared to be confused. The area that the playground is located in has a low surrounding fence, which made Child #1 visible to the public that walk pass and reside near the playground. Child #1 was left alone and unsupervised in an environment that exposed her to potential harm. a. Child #1 was observed on the playground by a parent entering the facility to pick up another child. The parent immediately notified staff in the office that there was a child left alone on the playground. The Staff were unaware that Child #1 was on the playground. The parent for child #1 arrived at the facility simultaneously and ran out to the playground finding child#1 on the playground. Child #1 ran to the parent "crying and upset". 2. During interviews Staff # 1 stated that she did not verify that all the children on the playground were accounted for before entering back into the facility. a. Staff #1 was unaware that Child#1 was left on the playground and had documented that Child #1 was no longer present at the program for the day, although Child #1 was still in the care of the center.

Description: Based on video review and interview, a child under 10 years of age was not always within actual sight and sound supervision of staff. Evidence: 1. On July 1, 2019 a four year old child was left unsupervised on the outdoor playground for 20 minutes. 2. Staff #1 confirmed that the Child#1 was left alone and unsupervised for 20 minutes on July 11, 2019. 3. Video that was reviewed showed Child #1 left alone on the outdoor playground.
