Technical Assistance:
Discussed aquarium safety, playground safety, and ongoing staff training.

Comments:
An unannounced monitoring inspection was conducted today from 10:00 am to 1:45 pm. There were 62 children (3-6 years) directly supervised by 6 staff. The physical plant, 5 staff's records, 6 children?s records, medications, menu, evacuation drills and emergency supplies were inspected. Children were observed participating in center activities like spooning, tracing, drawing and using Montessori materials. Hand washing procedures were also observed. There was an abundant supply of materials for the children. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review, staff records were incomplete. Evidence: Staff #1, #2, #3, #4, #5 and #6 did not have any information about any health problems which may interfere with fulfilling their job's responsibilities.

Plan of Correction: Will create an addendum to include the information.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition. Evidence: 1) In the preschool playground there were three nails/screws protruding from the wood fence and the block holder. 2) One of three slides had a gap at the top which causes a strangulation/entanglement hazard. 3) The hand washing sink in the blue room was leaking water in the cabinet and onto the floor. 4) The hand washing sink in the white room was leaking in the cabinet.

Plan of Correction: I will try and fix the screws myself. I will ask the handyman to fix the other areas.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method. Evidence: The storage closet in the hall where cleaning products were stored had a broken lock.

Plan of Correction: I am going to have the handyman fix it.

Standard #: 22VAC40-185-290-3
Description: Based on observation, electrical outlets did not have protective covers. Evidence: In the multipurpose room there was one electrical outlet that did not have a child-proof protective cover.

Plan of Correction: Outlet was covered immediately during inspection.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the resilient surfacing did not comply with minimum safety standards in the fall zones. Evidence: The preschool playground resilient material (mulch) did not meet minimum safety depth of 6 inches.

Plan of Correction: The mulch should be delivered today.

Standard #: 22VAC40-185-350-C
Description: Based on observation, the staff-to-children ratio applicable to the youngest child in the group was not being maintained Evidence: In the blue room, ages 3-6 years, there were 24 children with 2 staff (1:12 ratio). The ratio for this mixed age group is 1:10, based on the youngest age in the group (9, 3-year-olds)

Plan of Correction: Until we hire another person, the assistant administrator will be in the classroom.

Standard #: 22VAC40-185-540-E
Description: Based on observation, one working, battery-operated radio was not available. Evidence: Batteries for the emergency radio were not operating.

Plan of Correction: Batteries were replaced immediately during this inspection.
