A complaint investigation was begun on July 16, 2019 in response to our office receiving information regarding a lack of supervision.
During the investigation, staff interviews were conducted, records and documents were reviewed. Based on the preponderance of evidence, the complaint is valid. Standard #: 22VAC40-185-40-E Complaint related: Yes Description: Based on staff interviews and a review of records, staff did not follow the center's own policies and procedures.
Evidence:
1. The center's "Transition Tracking Policy", states that teachers are required to use the We Care Transition Tracking Sheet to account for children in their care to conduct a Name to Face Roll Call whenever a transition through a door or gate occurs followed by a head count.
2. On March 22, 2019 at approximately 5:30pm, Staff 1 took the classroom children from the 2's -3 room to the 2's- 2 room. Staff 1 stated she did not sign the children into the classroom, as she needed a bathroom break. No Name to Face Roll Call or a head count was conducted. Staff 1 did not sign into or out of the room.
3. Child A (3 years old) was left alone an unsupervised in a closed classroom for approximately 5 minutes until the child was found by their parent and another center staff member.Plan of Correction: Follow plan submitted on 7/2/19
Standard #: 22VAC40-185-340-F Complaint related: Yes Description: Based on multiple staff interviews, a child under the age of 10 years was not kept within actual sight and sound supervision of staff.
Evidence:
At approximately 5:40pm on March 22, 2019 Child A (3 years old) was found alone in the Preschool-3 classroom by a staff member and the child's parent. When the parent went to pick up their child from the 2's-2 room, staff discovered they did not have the child in the room and call went out over the intercom for help. Staff searched with the parent and found the child in the closed PS3 playing with toys on the carpet. The 2's groups was checked into the 2's-2 room at 5:35pm.Plan of Correction: Follow plan submitted on 7/2/2019
