Standard #: 22VAC40-665-790-F Description: Based on a review of documentation and interview on 08/08/2019, the vendor did not ensure that a child with a diagnosed food allergy shall not be served any food identified in the written care plan required in 22VAC40-65-520 B 11.
1) The written care plan in the record for Child #1 (two-year-old) indicates the child has a diagnosed food allergy to wheat and eggs.
2) During interview, staff reported that on 07/08/2019, Child #1 was served and ate saltine crackers at approximately 3:00pm.
3) The licensing inspector reviewed the list of ingredients listed for the saltine crackers that Child #1 was served on 07/08/2019. The list of ingredients indicate that the crackers contain wheat.
4) Staff interviewed acknowledged Child #1 is allergic to the ingredient listed.
Plan of Correction: Per the Center: "All staff have been retrained on proper allergy prevention. All staff have been retrained on how to read an allergy list."
