An announced initial inspection was conducted with the center director from 11:00 a.m. to 4:30 p.m. Classrooms were observed and are well organized and equipped with age appropriate supplies and furnishings. Childcare areas were measured and the facility can accommodate the requested capacity of 140 children. Both playgrounds were observed and inspected. The large playground can accommodate 11 children. The outdoor grassy area can accommodate 58 children at any one time. The infant/toddler playground can accommodate 16 children. Five staff records were reviewed. Two staff have been trained in Medication Administration Training (MAT). The emergency preparedness plan and supplies were reviewed. Standard #: 22VAC40-185-330-B Description: Based on observation, not all playground equipment met the minimum safety standards for resilient surfacing. Evidence: On the large playground, the use zone for the large slides does not meet the required eight feet use zone and measures at 7 feet 3 inches at the front of the slide chute. Plan of Correction: The slides will be off limits until the slides meet the requirement.
Standard #: 63.2(17)-1721.1-B-2 Description: Based on review of documentation, the center failed to obtain required finger print background checks for all agents of the LLC. Evidence: Agent #4 did not have documentation of satisfactory results of the national fingerprint background check. Plan of Correction: Agent #4 immediately scheduled a fingerprint background check appointment and satisfactory results were received 9/4/2019.
Standard #: 63.2(17)-1721.1-B-3 Description: Based on review of documentation, the center failed to obtain required out of state child abuse and neglect registry search background checks for all agents of the LLC. Evidence: A Maryland child abuse and neglect registry search was notarized 5/2/2019 for Agent #4. There was no documentation of satisfactory results of an out of state child abuse and neglect registry search for Maryland or documentation that the center followed up on the background check within 45 days. Plan of Correction: Agent #4 contacted Maryland regarding the background check and it has been received and results will be provided as soon as possible.
