63.2(17) License & Registration Procedures

Comments:
An unannounced renewal inspection was conducted today, September 4, 2019 to determine the center's compliance with licensing standards. The Inspector arrived at 1:30 pm and concluded the inspection at 4:00 pm. The center owner and administrator participated in the inspection. Upon the inspectors arrival, children in the infant room were napping while a group of infant and toddlers played with balls in the breeze way. Children in the the toddler classroom were getting up from nap, having bathroom breaks and washing there hands. Toddlers also were observed choosing work task that consisted of puzzles, manipulatives, books and snack. Later the infants were observed playing on the carpet and reading stories while the toddlers did finger plays and sang songs. Staff interacted in a positive manner. All areas of the facility were inspected to include two classrooms, hallway, bathroom and out door playground. Required information was posted to include: license, last inspection notice, emergency number, evacuation maps/procedures and daily schedules. The facility has age and stage appropriate supplies and materials. The first aid kit and non medical emergency supplies were inspected and found complete. The inspector interacted with staff and children in care.

Five staff records and five children's records were reviewed. (Records for the Board members were also reviewed.)
Currently medication is not being administered. Multiple staff have current CPR, First Aid, Daily Heath Observation training and MAT.
The most recent evacuation drill was documented on 08/13/2019. The two required shelter-in-place drills were completed on 01/03/2019 and 08/13/2019.

Today's Census:
13 children, 5 staff

The violations cited during the licensure period were checked for corrections. There were no repeat violations cited during today's inspection. Violations:
Standard #: 22VAC40-185-160-C
Description: Based on review of five staff records, the center did not obtain the results of a follow up tuberculosis screening at least every two years from the date of the first initial screening or testing for one staff member.

Evidence:

Documentation indicated the most recent tuberculosis screening for staff #2 was 08/22/2017, exceeding the two year allowance to resubmit a current tuberculosis screening.

Plan of Correction: Per the Administrator: We will have the staff member obtain an updated tuberculosis screening for the record.

Standard #: 22VAC40-185-70-A
Description: Based on review of five staff records, the center did not obtain all required documentation for one staff record.

Evidence:

The record of staff #3, hired 05/31/2019, did not have documentation of the dates of contact for the two references as required.

Plan of Correction: Per the Administrator: We will review with administration all information that is required and ensure dates references are conducted are documented.

Standard #: 22VAC40-185-350-D
Description: Based on observation and interview, the center did not ensure during the designated sleep period, the ratio of staff to children was doubled the number of children to each staff only for children 16 months old through the age of eligibility to attend public school.

Evidence:

1. Child #1, 15 months in age, was observed at approximately 1:30 in the toddler classroom during the scheduled rest period along with seven additional children and one staff. All eight children were awake.
2. The ratio of staff to children may not be doubled during rest periods for children under the age of 16 months.

Plan of Correction: Per the Administrator: We will have the required number of staff in the classroom to meet the ratio. Two additional staff returned to the toddler classroom while the inspector was on site.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review of five staff records, the center did not request a search of out of state founded complaints of child abuse or neglect within 30 days of beginning employment for two staff members who have lived outside of Virginia within the last five years.

Evidence:

1. Documentation for staff #1 (hired 04/15/2019) and staff #3 (hired 05/31/2019) indicated the staff members had lived outside of Virginia within the last five years. The records did not contain documentation of a request for founded complaints of child abuse from the states the staff members indicated they had lived in with the last five years. Both staff #1 and staff #3 were observed working at the center at the time of the inspection.
2. Administration stated an out of state child abuse search for staff #1 and staff #3 had not been requested.

Plan of Correction: Per the Administrator: I have already pulled up the forms to be completed while he inspector was on site. I will submit the completed request forms today and send a copy to the department with the required ten days.
