An unannounced monitoring inspection was conducted today from 7:15am-8:40am. There were 42 children directly supervised by 4 staff. The physical plant, programming, menus, 4 staff records, 5 children?s records, 8 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed doing puzzles, coloring, learning about the moon and playing silent speed ball. Positive interactions between children and staff and nice transitions between activities were observed during this inspection. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A Description: Based on review of 5 children's records, 1 child's record did not contain all required information.
Evidence:
1. Child #1's file was missing 1 of 2 emergency contacts to include name, address, and telephone number.Plan of Correction: I'm going to call his mother and have her update the second emergency contact right now.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on review of 4 staff records, there was no documentation an out of state Child Abuse and Neglect Central Registry search request was submitted by the end of the 30th day of employment for a staff who has resided outside the state of Virginia within the last 5 years.
Evidence:
1. Staff #4 (Date of Hire: 2/25/19) indicated on a Sworn Disclosure Statement dated 2/25/19 that she has resided outside the state of Virginia within the last 5 years. Staff #4's file did not contain documentation of an out of state Child Abuse and Neglect Central Registry search request.Plan of Correction: I will obtain the paperwork today from my area manager. She has told me they have the out of state check in the main office.
