Comments:
An unannounced renewal inspection was conducted today between 10:00 am and 3:30 pm. There were 72 children present with a staff of 22. The children were observed in circle time, working on art projects, hand washing and eating lunch. Lunch consisted of H Tuna, cheese, baby carrots, honeydew melon and milk. The entire physical plant, playgrounds, children's records, staff records, incident/accident logs, evacuation logs, emergency plans, menu's, emergency supplies and medications were inspected. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of 5 children's records, the center failed to ensure that children's records are maintained and up to date.

Evidence:
1) Child # 1 and #2's records do not have emergency pick-up persons who are local.
2) Child #4's record does not have documentation of addresses for either of the emergency contacts.

Plan of Correction: I will contact parents today and get the information for the children's files.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that the areas and equipment of the center shall be maintained in a clean, safe and operable. condition.

Evidence:
1) In the Jr. Preschool 1, there is a hole in the wall next to the bathroom door.
2) In the Infant 1 classroom, the shape mat is ripped at the corner.
3) In the Pre-Kindergarten classroom, the baseboard near door and the baseboard around the large alphabet carpet has chipped paint.

Plan of Correction: We will make necessary repairs to wall and with paint.
We will order a new shape mat for the Infant 1 classroom.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the center failed to ensure that electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.

Evidence:

In the Toddler 1 classroom, 2 electrical outlets were missing protective covers.

Plan of Correction: I will replace outlet covers.

Standard #: 22VAC40-185-520-C
Description: Based on observation, diaper ointment or creams in four classrooms are accessible to children,

Evidence:
In the Jr. Preschool 1, Jr. Preschool 2, Toddler 1 and Toddler 2 classrooms the diaper creams are stored under the changing tables and accessible to children.

Plan of Correction: I will notify the owner and we will store diaper cream out of reach of children.
