63.2(17) License & Registration Procedures
An unannounced monitoring inspection was conducted today from 6:55am-8:30am. There were 34 children directly supervised by 3 staff. The physical plant, programming, menus, 2 staff records, 7 children?s records, 5 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed reading books, playing Jenga and Mancala, doing origami, playing a group game with balloons, and doing an art project. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A Description: Based on review of 7 children's records, 4 children's records did not contain all required information.
1. Child #4 (Date of Enrollment: 8/29/19), Child #5 (Date of Enrollment: 9/3/19), and Child #6 (Date of Enrollment: 8/26/19) did not have immunization records on file.
2. Child #7's file is missing 1 of 2 emergency contacts' addresses.
Plan of Correction: I will obtain all required information from parents.
Standard #: 22VAC40-185-70-A Description: Based on staff record review, there was not a record on site for each staff present.
1. Staff #3 was present at the beginning of the inspection and does not have a file on site.
Plan of Correction: I will obtain the staff's file from my Area Manager before the staff returns.
Standard #: 22VAC40-191-60-B Description: Based on staff record review, one staff present did not have documentation of a Sworn Disclosure Statement completed prior to first day of employment.
1. Staff #3 (Date of Hire: 9/5/19) did not have a Sworn Disclosure Statement on file.
Plan of Correction: I will obtain the Sworn Disclosure Statement from my Area Manager before the staff returns.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on staff record review, one staff present did not have documentation of fingerprinting results prior to employment on file.
1. Staff #3 (Date of Hire: 9/5/19) was present at the beginning of the inspection and did not have fingerprinting results on file.
