An unannounced monitoring inspection was completed on 09/03/2019. There were 16 children, ages 3 years-4 years, 2 staff providing direct care and supervision and 2 additional support staff present during the inspection. The inspector reviewed 5 children's records, the on-site staff record requirements for 4 staff; the complete staff records will be reviewed at the corporate office on a later date. The provider stated that medication is not being administered at this time. Children were observed in the following activities: during free choice/center play, during story time, participating in music and movement, during lunch and going down for rest time. The inspector reviewed the bus, the emergency evacuation and shelter-in-place plans and the emergency supplies during the inspection. The inspector discussed children's records, children's allergies and updating documents with the staff. Standard #: 22VAC40-185-60-A Description: Based on record review and discussion with the staff, the facility failed to ensure that the separate record for each child enrolled shall contain all of the elements as required by the standards.
Evidence:
1. The record for Child #3 did not contain the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached; the address for the second designated person was incomplete.
2. The record for Child #5 did not contain the first date of attendance; Child #5 was present during the 09/03/2019 inspection.Plan of Correction: Phone calls and/or letters to parents will be made to get the missing information.
Standard #: 22VAC40-185-290-3 Description: Based on observations and discussion with the staff, the facility failed to ensure that in areas used by children of preschool age or younger, electrical outlets shall have protective covers.
Evidence: There was an uncovered electrical outlet beside of the television; preschool aged children were observed in this area during the 09/03/2019 inspection.Plan of Correction: The outlet was covered during the inspection.
Standard #: 22VAC40-185-540-D Description: Based on observations and discussion with the staff, the facility failed to ensure that all emergency supplies shall be available on field trips.
Evidence: There was not an ice pack or cooling agent on the bus used daily.Plan of Correction: An email will be sent to the Health and Disability Specialist.
