Comments:
An unannounced monitoring inspection was completed on 09/03/2019. . There were 22 children, ages 4 years-12 years, and 4 staff present during the inspection. The inspector reviewed 5 children's records, 5 staff records and 2 medications during the inspection. Children were observed in the following activities: arriving to the program, during homework time, having an afternoon snack and going to the gym to play. The inspector reviewed the emergency preparedness plans and the emergency supplies during the inspection. The inspector discussed subsidy requirements, transportation and staff records with the staff. The inspector arrived for the inspection at 2:00 p.m. and departed at 5:00 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review and discussion with the staff, the facility failed to ensure that the separate record for each child enrolled shall contain all of the elements as required by the standards.

Evidence:
1. The record for Child #2 did not contain the name of any additional programs or schools that the child in concurrently attending.
2. The record for Child #5 did not contain information regarding allergies and intolerance to food, medication, or any other substances, and actions to take in an emergency and the name of any additional programs or schools that the child in concurrently attending.

Plan of Correction: The information will be obtained and placed in the record.

Standard #: 22VAC40-185-550-C
Description: Based on observations and discussion with the staff, the facility failed to ensure that emergency evacuation and shelter-in-place procedures/maps shall be posted in a location conspicuous to staff and children on each floor of the building.

Evidence: The procedures for emergency evacuations and shelter-in-place posted during the 09/04/2019 inspection.

Plan of Correction: The procedures were posted during the inspection.
