Technical Assistance:
Technical assistance was provided regarding the renewal process.

Comments:
An unannounced renewal inspection was conducted today from 8:45am-12:00pm. There were 68 children (infants-5yrs.) supervised by 14 direct care staff. The physical plant, 10 staff records, 15 children?s records, 9 medications, evacuation drills, injury reports, and policies were inspected. Children were observed participating in group play, reading books, and playing outside. Diapering and hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Standard #: 22VAC40-185-130-B
Description: Based on review of 10 children's records, 1 did not have documentation of an updated immunization record every 6 months for a child under the age of 2.
Evidence:
Child #1, under the age of 2, does not have an updated immunization record, last update was 1/16/19.

Plan of Correction: I already contacted mom, she will drop it off today.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas of the building, inside and out are not being maintained in a clean, safe and operable condition.
Evidence:
In the Pre K classroom, peeling paint was observed in the window sill.
In the Infant 2 classroom, chipped and peeling paint was observed by the back door
On the Preschool playground there are roots that are above the ground causing a tripping hazard, rocks protruding out of the mulch, that cause a tripping hazard, the wood gazebo has rotting wood that is splintering, lifting and separating causing a possible finger entrapment.
On the Toddler playground there are 2 green plugs missing, exposing finger size holes, and the top railing is splitting and splintering.

Plan of Correction: Work orders have been submitted for all repairs.

Standard #: 22VAC40-185-370
Description: Based on observation, 1 out of 2 infants are not have tummy time recorded.
Evidence:
Child #3 does not have documentation of 30 minutes of tummy time recorded daily.

Plan of Correction: We usually record it, we will make sure it is done daily.

Standard #: 22VAC40-185-510-A
Description: Based on review of 9 medication and medication authorization, 1 does not have the correct medication.
Evidence:
Child #2 had a generic medication on site, the authorization is written for a name brand.

Plan of Correction: I have contacted the parent to bring in the name brand.
