Comments:
An unannounced complaint investigation was conducted on 09/03/2019 from 3:00 pm - 5:45 pm in response to a complaint that was received on 08/08/2019, regarding emotional and physical support, and hand washing procedures. The Director was present during the inspection and for the exit interview, at which time the inspection findings were reviewed. Observations were made, records were reviewed, and interviews with administration were conducted. Based on the information gathered, the preponderance of evidence supports the allegations; therefore, the complaint was determined to be valid. The complaint related violations and non-related violations are identified on the Violation Notice. Complaint related: No
Description: Based on record review, the center did not obtain a current tuberculosis (TB) test/screening at least every two years from the date of their last TB screening/test.

Evidence:
Staff#2's record contains a TB test/screening that was last updated on 10/1/2015.

Plan of Correction: We have requested a copy of the up-to-date TB test.

Complaint related: Yes
Description: Based on interview, in order to promote the child's emotional and social well-being a staff person did not provide needed comfort and support to a child in care.

Evidence:
On 8/8/19, at approximately 10:30 am Staff#1 was observed on the playground alone with 5 children. The staff person was observed sitting at the picnic table with 4 children while Child#1 (16 months old) was observed playing with a riding toy between the gate that leads to the parking lot and the A/C unit. Child#1 was observed running over themselves with the riding toy. Child#1 fell and was observed and heard screaming for approximately a minute and a half under the toy. Staff#1 was observed sitting at the table and telling the child: "come here". Child#1 rolled the toy off of them, grabbed the fence, and pulled themselves up. Child#1 was observed crying and trying to walk to the picnic table. When the child made it to half way between the picnic table and the toy, Staff#1 was observed getting up to attend to the child.

Plan of Correction: The staff member observed was terminated immediately.

Description: Based on interview statement, hand washing procedures were not followed before meals and after a diaper change.

Evidence:
On 8/8/19, Staff#2 was observed changing the diaper of Child#2 (approx. 9 months). After changing the diaper, Staff#2 was observed removing their gloves, picking up the child and placing them in a highchair. Staff#2 then walked to the refrigerator, grabbed Child#2's bottle and handed it to them without washing their hands. Child#2 did not have their hands washed after diapering or before eating.

Plan of Correction: Staff member reviewed infant diaper changing policy and hand washing policy.

Description: Based on record review, the center did not obtain a fingerprint based background check for a staff member prior to their first day of employment.

Evidence:
Staff#2's record does not contain a fingerprint result required before their first day of employment. The staff's first day of employment was documented as 5/15/2017.

Plan of Correction: We will complete it before seasonal staff member returns to work.
