Technical Assistance:
Discussed obtaining a note from a physician prior to changing an infant's sleep position.

Comments:
An unannounced monitoring inspection was conducted today from 11:30am-1:00pm. There were 82 children present directly supervised by 16 staff. The physical plant, 5 staff records, 5 children?s records, staff training, evacuation drills, injury reports, emergency supplies, and policies were inspected. Infants were observed participating in floor play with caregivers and being fed. Older classes were finishing lunch consisting of: Beef ravioli, green beans, applesauce, toast and milk. Hand-washing and sanitation procedures were also observed. No medications were on site for inspection today. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Description: Based on record review, one of five staff records reviewed did not contain documentation of a negative tuberculosis screening within 21 days after employment.
Evidence:
Staff 1's record did not contain a negative Tuberculosis screening.

Plan of Correction: We will ask the staff member to provide a negative screening document.

Standard #: 22VAC40-185-70-A
Description: Based on record review, one of five staff records reviewed was incomplete.
Evidence:
Staff 1's record did not contain documentation that two references were obtained.

Plan of Correction: We will add the references to the staff member's file.

Standard #: 22VAC40-185-270-A
Description: Based on observation, not all areas inside the center were maintained in a clean, safe and operable condition.
Evidence:
The wall above the bathroom sink in classroom C was observed to be peeling and accumulating dirt and grime that cannot be properly cleaned as a result of the damaged wall.

Plan of Correction: We will put in a work order today to fix the wall above the sink.
