Comments:
An unannounced Renewal Inspection was conductedon 9/4/2019 from 2:55pm to 4:56pm. There were 46 school-agers directly supervised by 3 staff. The physical plant, 1 staff record, 5 children?s records, 2 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in homework, study time, reading books, playing outside and group activities. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Description: Based on observation, the facility failed to ensure that hazardous substances such as cleaning materials are kept in a locked place using a safe locking method that prevents access by children. Evidence: Observed in the unlocked and open staff office, Oxivir and Bath Mat, sprays accessible to children on low shelves.

Plan of Correction: Corrected at the time of the visit.

Standard #: 63.2(17)-1721.1-B-2
Description: Based on review, the facility failed to ensure that staff have documentation of submitting to fingerprinting prior to being hired. Evidence: Reviewed 1 staff record and found that they were missing documentation of completing fingerprinting prior to being hired.

Plan of Correction: Will obtain documentation of fingerprinting.
