Technical Assistance:
**Discussed that since the diaper changing areas for the toddlers and 2s classrooms are not inside of the classrooms, but in a bathroom down the hall, staff and the director will need to ensure that ratios and supervision are maintained at all times when staff need to leave the classrooms to change diapers. **Please send the inspector documentation of the missing central registry check report, staff documentation of MAT training and documentation of liability insurance as soon as it is received. The license cannot be approved without the owner's central registry check being completed and documentation sent to the inspector for review and verification.

Comments:
An Announced Initial Inspection was conducted today from 10:05am to 12:30pm. There were no children in care. The physical plant, 2 staff records, no children?s records, no children's medications and medication authorization records, evacuation drills or injury reports. Emergency supplies, and policies were inspected. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in, CPR and First Aid, as well as DHO training. Staff will need to ensure that complete files will be transferred to the facility when staff transfer from the other locations. The center was clean and organized. Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility failed to ensure that the areas and equipment of the center, inside and outside shall be maintained in a clean, safe and operable condition. Evidence: 1) Observed a railing post that was warped and pulling away from the walkway exposing 2 nails that are accessible to children. 2) Observed 3 loose screws along the fence of the infants playground. 3) Observed 1 screw on the ground on the walkway from the infant and toddler playground. 4) Observed in the Art and Science Room, window blind cords accessible to children.

Plan of Correction: Corrected at the time of the visit, and the handrail nails will be corrected tomorrow.

Description: Based on observation, the facility failed to ensure that medication was kept in a locked place using a safe locking method that prevents access by children. Evidence: Observed pain medication (aspirin, ibuprofen) in an unlocked first aid kit in the director's office.

Description: Based on observation, the facility failed to ensure that the first aid kit on the second level had the minimum required items. Evidence: Observed that the second level first aid kit was missing an ice pack and a first aid manual.

Standard #: 22VAC40-185-550-C
Description: Based on review, the facility failed to ensure that emergency evacuation and shelter-in-place procedures/maps are posted in a location conspicuous to staff and children on each floor of each building. Evidence: Reviewed the maps in the building and found that they did not have documentation of the shelter-in-place location.

Plan of Correction: Will be corrected by tomorrow.

Standard #: 22VAC40-185-550-F
Description: Based on observation, the facility failed to ensure that emergency numbers (911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center) shall be posted in a visible place at each telephone. Evidence: Observed that there were no emergency numbers posted in the classrooms near the phones.

Plan of Correction: Will be completed today.

Standard #: 63.2(17)-1721.1-B-3
Description: Based on review, the facility failed to ensure that the director/co-owner of the facility has documentation of results of a central registry check completed in Virginia. Evidence: Reviewed the owner/director's records and found that they had documentation of a central registry check being submitted but did not have documentation of results on file.

Plan of Correction: Will obtain missing documentation by Monday.
