Comments:
The licensing inspector conducted a unannounced renewal inspection Tuesday, August 27, 2019 from 10:00 am to 1:20 pm. The children were observed engaged in a variety of activities. The center was having a center wide end of summer party. The infants were observed playing with toys on the floor with their teachers, and being fed.
Ten children?s records and Ten employee records were reviewed during this inspection.
Medication is being administered, however none was present
The center's first aid kit and emergency supplies were inspected.

Total enrollment today: 136
Today, the following child to staff ratios were observed:
Infants I?12:3

PROVIDER:
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 22VAC40-185-160-C
Description: Based on a review of staff records, and staff interview on 08/27/2019, the center did not ensure that at least every two years staff members shall obtain and submit the results of a follow-up tuberculosis(tb) screening.

EVIDENCE:

1. The most recent tb screening for Staff #2 is dated 05/24/2017.
2. Staff stated that the tb screening was done, but didn't have the documentation.

Description: Based on a review of children's records on 08/27/2019, the center did not ensure that one of the ten children's records reviewed contained the required information.
