Comments:
An announced initial inspection was conducted on September 4, 2019 to determine compliance with licensing standards. The inspection was conducted with the owners and program director. There were three children present at the time of the initial inspection; center operating under Exemption 63.2.1715.B.3. The facility plans to provide care to children three years to twelve years. Measurements of the facility were made to determine the capacity of 83 children. Classrooms are equipped with a variety of age appropriate equipment and supplies including materials for gross motor, art, literacy, and dramatic play. Six staff files and four administrator files were reviewed - please see Supplemental Summary for details. Four staff have documentation of Medication Administration Training. Several staff have documentation of first aid and CPR certification. Policies and procedures were reviewed. A conditional (6 month) license will be recommended. An unannounced follow up inspection will be conducted within 60 days to determine compliance with standards not determined at the initial inspection. Standard #: 22VAC40-185-70-A
Description: Based on review of six staff records, the facility failed to obtain two references as to character and reputation as well as competency prior to employment. Evidence: there were no documented references for Staff 2 (date of hire 8/22/2019).

Plan of Correction: Center administration will contact two references for Staff 2, and will check references for each new staff prior to hire.

Standard #: 22VAC40-185-290-3
Description: Based on observation of the building, there were no protective covers on electrical outlets in the areas used by preschool age children and younger. Evidence: there were no electrical outlets in the preschool classroom, the gym, the hallways, or the indoor playground area.

Plan of Correction: Electrical outlets will be covered with protective covers.

Standard #: 22VAC40-185-420-A
Description: Based on review of the parent handbook, the facility failed to provide in writing all required policies. Evidence: the parent handbook does not include the following policies: policy on when a child is not picked up during an emergency situation including inclement weather, the center's policy on administering sunscreen or insect repellent, established lines of authority for staff, policy for reporting child abuse as required by the Code of Virginia, or policy for communicating an emergency situation with parents.

Plan of Correction: The parent handbook will be revised and a new copy will be furnished to parents.

Standard #: 22VAC40-185-550-A
Description: Based on staff interview, the facility has not completed an emergency preparedness plan that addresses staff responsibility and facility readiness with respect to emergency evacuation and shelter in place. Evidence: staff stated that an emergency preparedness plan has not been developed.

Plan of Correction: An emergency preparedness plan will be developed and will consult with local authorities.
