A discussion was held on keeping the doors closed. Center had already posted a note on every door to "only open in case of an emergency."

An unannounced self reported inspection was conducted regarding concerns of child supervision. Statements were reviewed and interviews conducted with all involved parties. Standard #: 22VAC40-185-340-F Description: Based on review of staff statements and interview with management, staff did not always ensure the care and protection of a child.
Evidence:
Child #1 was not in sight and sound of staff for less than 1 minute.Plan of Correction: Staff involved have been terminated and all current staff has been retrained on child supervision.
