Technical Assistance:
A discussion was held on procedures for annual parental emergency contact information.

Comments:
An unannounced monitoring inspection was conducted today from 9:00am-11:30am. There were 98 children (infants-5yrs.) supervised by 23 direct care staff. The physical plant, 10 staff records, 7 children?s records, 6 medications, programming and policies were inspected. Children were observed participating in group play, music class, building, writing, going on a walk, playing outside, and eating snack. Diapering and hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Violations:
Standard #: 22VAC40-185-130-B
Description: Based on review of 7 children's records, 2 do not have documentation of updated immunizations every 6 months for children under the age of 2.
Evidence:
Children #1 and 2 do not have updated immunizations, last update was 7/2/18 and 9/25/18 respectively.

Plan of Correction: I will contact the parents again today.

Standard #: 22VAC40-185-160-C
Description: Based on review of 10 staff records, 2 do not have documentation of an updated tuberculosis (TB) screening every 2 years.
Evidence:
Staff #1 and 2 have documentation of expired TB screenings. Current TB screening expired 4/19 and 3/19 respectively.

Plan of Correction: We will have the updated TB screening results on file by next week.

Standard #: 22VAC40-185-290-3
Description: Based on observation, not all electrical outlets are covered with a protective cover.
Evidence:
The outlets along the wall in the common area upstairs are not all covered and are accessible to children.

Plan of Correction: Corrected during inspection, we will make sure to do a walk through every day to ensure they are all covered.

Standard #: 22VAC40-185-500-B
Description: Based on interview with 2 staff, the diapering area is not being cleaned and sanitized according to licensing standards.
Evidence:
Staff in the Infant 2 room are using lysol and water instead of soap and water, staff in the Preschool 2 room are using lysol and water instead of bleach and water. The correct procedure is to spray with a solution of soap and water, wipe dry, spray with a solution of bleach and water and allow to air dry.

Plan of Correction: Corrected during inspection, proper procedure with be reviewed again with each staff member.

Standard #: 22VAC40-185-510-N
Description: Based on review of 6 medications, 2 have expired medications that have not been renewed, picked up by the parent of disposed of by the center.
Evidence:
Children # 3 and 4 have a medication authorization that expired on 8/1/19, that has not been renewed, The medication has not been returned to the parent or disposed of by the center within 14 days of expiration.

Plan of Correction: We will have the parent get the authorization signed by physician.
