Technical Assistance:
The renewal application was reviewed.

Comments:
The inspector conducted an unannounced non-mandated inspection from 10:45 a.m until 1:00p.m. There were 78 infants and children in care with staff supervising at all times. The director, assistant director and cook were also present. Infants were observed napping, playing and being fed. Children were observed playing inside and outside, washing hands, during drop-off, lunch, and rest time. Nine staff records and 7 child records were reviewed. The violations from the last inspection have been corrected. Violations are identified on the violation notice. Please call your inspector at 276-698-9981 with any questions.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to keep all hazardous substances locked. Evidence: 1. The inspector observed cleaning supplies unlocked in the Infant 1 room, and in the preschool room. Both cabinets had locks on them but they were not locked. 2. The director observed the unlocked hazardous substances and locked them .

Plan of Correction: The director will remind staff to keep all hazardous substances locked at all times.

Standard #: 22VAC40-185-350-E-1
Description: The center did not maintain the required ratios. Evidence: 1. The inspector observed one staff person and five infants upon entering Infant Room 1. 2. The director also observed the situation and said that a child had just checked in and a staff person was being sent into the classroom, a second person entered the classroom within 5 minutes.

Plan of Correction: The director will review and improve the process for maintaining ratios.

Standard #: 22VAC40-185-510-A
Description: Based on record review and interview, the center did not have parental authorization to administer a medication. Evidence: 1. The inspector reviewed medication paperwork, parental authorization was missing for child #11.

Plan of Correction: The director is in the process of getting the authorization.

Standard #: 22VAC40-185-560-N
Description: The center allowed a child to drink while walking around. Evidence; 1. The inspector observed a child walking around drinking milk. 2. The director was also present to observe and instructed staff to not allow children to walk around while drinking or eating.

Plan of Correction: The director will remind staff that children are not allowed to walk around while drinking or eating.
