Technical Assistance:
Infants require daily outside time- weather permitting-370.3
Bleach water solutions must be made fresh daily.
For children younger than 3, diapering shall take place on a changing tale or counter top designated for changing- 500B3c
Make sure you are varying the time of day you do the monthly escape drills- the last 7 have been in the morning- 550E
Maintaining an accurate attendance in the classrooms is key in maintaining supervision of the children.
Please revise your injury/accident form to include a space where staff can document " future action to prevent recurrence"- 550M6
Snacks need to consist of two components- water is not a component however milk is.
Cots/Mats shall have linens consisting of a top and bottom cover or one piece covering which is open on three edges- 440A

Comments:
An unannounced monitoring inspection was conducted today with the administrative staff. Ninety three children were present with 15 staff providing supervision. Many of the children were on cots, while others were engaged in quiet reading with most of the infants crawling and toddling around in the classroom.
Six staff and children's files were reviewed as well as 4 medications for two children. A sample size of 10 injury accident reports were reviewed.

The center had reported a self reported situation involving sight and sound supervision.Interviews were conducted and there was one violation resulting from this investigation.

Violations:
Standard #: 22VAC40-185-80-A
Description: Based on observation and interview, it was determined that the center is not maintaining an accurate written record of children in attendance in the classrooms.
Evidence:
1) In classroom 103, 11 children were present however the attendance listed 13 children as being present.
2) In classroom 107, 9 children were present however the attendance listed 18 children as being present.
3) In the infant room, there were 6 children present however the attendance listed 1 child as being present.
4) In the Kindergarten classroom, 8 children were present however the attendance listed 20 children as being present.

Plan of Correction: We have retrained our staff in how to take attendance properly throughout the day as a child arrives at school immediately, is being transitioned into another classroom, or is being picked up. The Administration Staff is also doing more periodic checks at different times of the day to ensure that this is being done properly.

Description: Based on observation and interview, it was determined that the center is not maintained in an operable condition.
Evidence:
The step on container used to dispose of soiled diapers was broken as the foot pedal used to open the container was inoperable in classroom 103.

Plan of Correction: We have a new step operated trash can for room 103

Description: Based on observation, it was determined that hazardous substances were not kept locked.
Evidence:
The Licensing Inspector (LI) observed the following:
1) In room 106 there were disinfectant wipes, disinfectant spray and glass cleaner unlocked in the cabinet;
2) In the Kitchen/ Laundry room there were laundry cleaner, fabric softener, multipurpose cleaner, glass cleaner and 10+ containers of disinfectant wipes, carpet cleaner stored unlocked on shelves and below the sink;
3) In the Kindergarten classroom there were disinfectant wipes, a spray bottle of bleach/water solution and disinfectant spray unlocked above the sink

Plan of Correction: All staff have been retrained on the importance of keeping all hazardous substances being locked up at all times and out of the reach of children.

All of hazardous substances in the kitchen have been locked up and are out of the reach of children

Standard #: 22VAC40-185-340-A
Description: Based on interviews, it was determined that the center failed to ensure the care, protection and guidance of a children in care.
Evidence:
Child A left the two year old classroom and was located in the kitchen. Staff A did not see the child leave the classroom nor did the teacher realize that the child was gone until the child was returned to the classroom by staff.

Plan of Correction: An Intensive Plan of Correction is being developed for this violation.

Standard #: 22VAC40-185-340-F
Description: Based on interviews, it was determined that a child under 10 years of age was not always within actual sight and sound supervision of staff.
Evidence:
On 8/14/2019 at approximately 8:45 a.m., child A, 2 year of age was found by a staff member standing in the kitchen alone next to the hand washing sink. The kitchen is located next door to the child's classroom. Staff A was not aware that the child had left the classroom therefore the amount of time the child was out of sight and sound supervision was not known.

Plan of Correction: An Intensive Plan of Correction is being developed for this violation.
