A complaint investigation was completed today from 7:35am-8:30am. The complaint was received by the Northern Licensing Office on 9/4/19 regarding confidentiality and children's records. Based on staff interview, review of documentation, and review of children's records, a preponderance of evidence supports the allegations regarding confidentiality and children's records. Standard #: 22VAC40-185-50-A Complaint related: Yes Description: Based on documentation review, children's information was not treated confidentially.
Evidence:
1. On 9/3/19 Staff #1 sent an email to parents of children attending the program which contained a list of 17 children's names who have incomplete files.Plan of Correction: I will keep children's information confidential and I will not do this again.
Standard #: 22VAC40-185-60-A Complaint related: No Description: Based on review of 4 children's records, 2 records did not contain all required information.
Evidence:
1. Child #3's file was missing 2 complete emergency contacts. The two listed emergency contacts had telephone numbers listed, however, they were missing addresses.
2. Child #4's file was missing an address for 1 of 2 emergency contacts.Plan of Correction: I will speak to the parents and obtain the missing information.
