Technical assistance was provided regarding confidential posting of allergy information and consultation was made regarding playground equipment.
- Comments:

An unannounced monitoring inspection was conducted today from 9:25am through 10:50am. There were 24 children directly supervised by 3 staff. The physical plant, outdoor playground area, programming, menu, 3 staff records, 5 children's records, 5 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed doing arts and crafts, playing with blocks, trucks, and playdough, participating in circle time, washing hands, and having snack which consisted of carrots, teddy grahams, and apple juice. Positive interactions between children and staff were observed during this inspection. All staff on site are current in First Aid/CPR certification and two staff are MAT (Medication Administration Training) certified. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-130-A Description: Based on review of 5 children's records, 1 child's record did not contain documentation of immunizations prior to the first day of attendance.
Evidence:
1. Child #2 (Start Date: 9/5/19) was present during the inspection and did not have immunizations on file.Plan of Correction: I will get the immunizations from the parent before the child returns to school.
