Technical Assistance:
A discussion was held on the overall cleanliness of the facility. There are areas that need a good cleaning or repainting, an example is the air vent in the Pre K 2 classroom is covered with a layer of thick dust, and chipped and peeling paint was observed throughout the center.

Comments:
An unannounced monitoring inspection was conducted today from 10:00am-1:30pm. There were 106 children (infants-5yrs.) supervised by 14 direct care staff. The physical plant, 7 staff records, 5 children?s records,3 medications, evacuation drills, emergency supplies, programming, and policies were inspected. Children were observed participating in group play, reading books, story time, singing, and eating lunch. Diapering and hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Standard #: 22VAC40-185-130-B
Description: Based on review of 5 children's records, one is incomplete and does not have documentation of an updated immunization record every six months for a child under the age of two.
Evidence:
Child # 3, does not have documentation of an updated immunization record, last record was 11/8/18.

Plan of Correction: I already contacted the parent to get the update.

Standard #: 22VAC40-185-160-C
Description: Based on review of 7 staff records, 1 is incomplete and does not have documentation of an updated tuberculosis (TB) screening every two years.
Evidence:
Staff #1, date of hire 5/28/18, does not have documentation of an updated TB screening, current one expired 8/15/19.

Plan of Correction: She will have it done by the end of the week.

Standard #: 22VAC40-185-210-A
Description: Based on review of 7 of staff records, 1 is incomplete and does not have documentation of qualifications for the position.
Evidence:
Staff #2 does not have qualifications to be a lead teacher.

Plan of Correction: Staff #2 is currently enrolled in college earning her degree towards early childhood development, she will bring in her transcript.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas of the center are not being maintained in a clean, safe operable condition.
Evidence:
In the intermediates classroom, observed was peeling and chipped paint by the sink, bathroom door and cubbies.
In the Pre K classroom, observed was peeling and chipped paint by the sink and back door.
In the Pre K 2 classroom, the back restroom has cracks in the floor, causing a possible tripping hazard.

Plan of Correction: The maintenance man will be tomorrow

Standard #: 22VAC40-185-320-B
Description: Based on observation, not all restrooms are equipped with paper towels and soap.
Evidence:
The restroom in the toddler classroom did not have paper towels.
The restroom in the Pre K 2 classroom did not have soap.

Plan of Correction: Corrected during the inspection, we will make sure all restrooms have supplies.

Standard #: 22VAC40-185-500-A
Description: Based on observation, staff and children's hands were not washed after each diaper change. Children's hands were not washed before mealtime.
Evidence:
Staff did not wash their hands or the children's hands after a diaper change.
Children hands were not washed before sitting down for mealtime.

Plan of Correction: I have already spoken to each staff member about ensuring all children's hands are washed.

Standard #: 22VAC40-185-500-B
Description: Based on observation the diapering surface is not being cleaned and sanitized after each use. Surface is also being used for other purposes other than diapering.
Evidence:
The diapering surface in the infant classroom was sprayed with bleach and water after a diaper change.
The diapering surface in the toddlers classroom was not cleaned or sanitized after each diaper change.
The diapering surface in the beginners classroom had none diaper changing supplies on top of the surface. Items included: a pen, pad, and tissues.

Plan of Correction: We will review the policies and procedures again with all staff.

Standard #: 22VAC40-185-510-A
Description: Based on review of 3 medications, 1 does not have an authorization from a parent or physician.
Evidence:
Child # 2 has a medication on site without any authorization to administer.

Plan of Correction: I have already contacted the parent to get the form signed by the physician and her. In the future we will not accept any medication without the paperwork.

Standard #: 22VAC40-185-510-N
Description: Based on review of 3 medications, 1 medication had an expired medication authorization.
Evidence:
Child #1 has a medication on site, with an expired medication authorization. Form expired 7/24/19.

Plan of Correction: We will return the medication to the parent.
