Technical Assistance:
Discussed nap requirements for mat spacing and ages of children who are required to be offered a nap period.

The local public schools medication authorization form is not permitted because the wording on that form indicates that only school personnel is permitted to administer the medication.

Comments:
An unannounced monitoring inspection was conducted today. There were 65 children in care, with a total of 9 staff members assigned to their direct supervision. Children's records, staff records, staff training, background checks, children's medications, the physical plant, emergency preparedness items, children's nutrition, and classroom programming were inspected today.

The children were observed engaging in various age-appropriate classroom Montessori activities, playing outside, and eating lunch. The children were served ravioli with tomato sauce, peas,watermelon, and milk for lunch today.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that one record was not complete.
Evidence:
Child record B does not include the child/parent's home phone number.

Plan of Correction: The parent will be asked for the missing information during pick up today.

Standard #: 22VAC40-185-240-A
Description: Based on a review of new staff training documentation, it was determined that two staff did not complete all required components of First Day Orientation training.
Evidence:
Staff B and C have not completed "Recognizing Child Abuse and Neglect" training.

Plan of Correction: They will both complete the training this weekend.

Standard #: 22VAC40-185-510-A
Description: Based on a review of children's medications, it was determined that one authorization is not complete.
Evidence:
The medication authorization form for one child's long term medication does not include the parent's signature.

Plan of Correction: The parent will be asked to sign the form today at pick up.

Standard #: 22VAC40-185-510-C
Description: Based on a review of children's medications, it was determined that three authorization forms were on site without all required information.
Evidence:
Three children's long term medication authorization forms were on site without: the physician's signature; documentation from the physician indicating a period of time for which the authorization is effective; detailed physician instructions for administration of the medication

Plan of Correction: Each parent provided this information but it was on the public schools authorization form. Each parent will be asked to have the correct form completed by the physician.

Standard #: 22VAC40-185-510-G
Description: Based on a review of children's medications, it was determined that one prescription medication was on site without all required information.
Evidence:
One child's prescription medication was on site without the prescription label for the medication.

Plan of Correction: The parent will be asked to bring the correct medication box with the label attached.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on a review of staff background checks, it was determined that one required background search finding has not been obtained.
Evidence:
Staff C began employment on 8/5/19. Because this person has been employed for longer than 30 days, her Central Registry background search findings are required to be on site. The school does not have her findings report available for review by the Inspector.

Plan of Correction: The search request was submitted but we do not have the results. Central registry was contacted immediately during the inspection. Once the search findings are received, I will immediately forward them to the Inspector.

Standard #: 22VAC40-80-120-A-6
Description: Based on observations, it was determined that the school was not operating within the terms of the current license.
Evidence:
Upon arrival, the Inspector observed one infant present and in care in the Lotus classroom. During staff interviews it was determined that this infant is the child of one staff person present in the same classroom. Although this staff person is not required per staff:child ratios, this person is fully engaged as a supervising staff person in the classroom. In addition, the terms of the license indicate that only children between the ages of 2 1/2 - 12 years may be in care.

Plan of Correction: This a very short term agreement for the staff person to be able to return to work. It's a very short term situation, only for a couple of months, until the child is a little older and able to stay in care elsewhere.
Effective immediately, we will no longer allow the baby to be in care in the room. Instead, the baby will stay in one of the office spaces away from the classrooms within the direct care of an administrative staff person who is not on staff in any capacity in any of the classsrooms. The decision to allow the staff person to bring the baby was announced to all parents and explained as a very temporary situation.
