Comments:
An unannounced monitoring inspection was completed on 09/09/2019. . There were 22 children, ages 3 years-4 years, and 4 staff present during the inspection. The inspector reviewed 5 children's records, the on-site staff record requirements for 5 staff and 1 medication during the inspection. Children were observed in the following activities: during large motor play on the outdoor playground, during music and movement/learning activities, during lunch and going down for nap. The inspector reviewed the emergency supplies during the inspection. The inspector discussed hiring additional staff, playground standards and injury reports with the staff. The inspector arrived for the inspection at 10:40 a.m. and departed at 1:45 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review and discussion with the staff, the facility failed to ensure that the separate record for each child contains all of the elements as required by the standards.

Evidence: The record for Child #2 did not contain the following information: allergies, and intolerance to food, medication or any other substances, and actions to take in an emergency situation and the name of any additional programs or schools that the child is concurrently attending.

Plan of Correction: The information was obtained during the inspection.

Standard #: 22VAC40-185-330-B
Description: Based on observations, measurements and discussion with staff, the facility failed to ensure that where playground equipment is provided, the resilient surfacing shall comply with the minimum safety standards. The resilient surfacing shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use.

Evidence:
1. The shorter slide (the slide facing the building) on the newest piece of equipment measured 3 feet 5 inches tall; the required fall zone for a slide this height is 7 feet 5 inches. The fall zone for the slide measured 6 feet.
2. The beam for the swings on the newest piece of equipment measured 7 feet tall; the required fall zone for swings this height is 14 feet in the front and the back of the swings. The fall zone behind the swings (the side closest to the parking lot) measured 11 feet 6 inches.
3. The beam for the blue/red swings measured 6 feet 6 inches tall; the required fall zone for swings this height is 13 feet in the front and the back of the swings. The fall zone in front of the swings measured 11 feet and the fall zone in the back of the swings measured 10 feet.
4. The slide on the Step2 piece of playground equipment measured 2 feet 8 inches tall; the required fall zone for a slide this height is 6 feet 8 inches. The fall zone for this slide measured 6 feet 4 inches.
5. The tricycle piece of equipment did not have any resilient surfacing underneath of it; children were observed using this piece of equipment during the inspection.
6. Grass was visible throughout various areas of the resilient surfacing.

Plan of Correction: The site coordinator will get additional mulch to correct the fall zones.

Standard #: 22VAC40-185-330-C
Description: Based on observations and discussion with the staff, the facility failed to ensure that ground support shall be covered with materials that protect children from injury.

Evidence: There were visible ground supports on the newest piece of playground equipment as well as on the red/blue piece of playground equipment.

Plan of Correction: The ground supports will be covered with the additional mulch that is purchased.
