The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on July 25, 2019 regarding allegations of inadequate staffing and staff qualifications. The inspection was completed on Friday, August 16, 2019 from 7:55am to 9:50am. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details. Standard #: 22VAC40-185-70-A Complaint related: No Description: Based on staff interview on 08/16/2019, the center did not ensure that a staff record was kept for each staff person.
EVIDENCE:
1. Staff #3 was observed working at the center.
2. Staff #3 was identified as center staff.
3. Staff #1 stated that Staff #3 did not have a staff record.

Standard #: 22VAC40-185-340-C Complaint related: No Description: Based on observation and staff interviews on 08/16/2019, the center did not ensure that during the stated hours of operation when one or more children are present, there always shall be on the premises one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee.
EVIDENCE:
1. When the inspector arrived at the center at 7:55am there was one staff and two children present at the center. There were not any other persons in the building. Staff arrived at the center at 8:10am
2. Staff stated that in the mornings starting at 7am there is only one staff or volunteer in the building by themselves with at the most five children.
