Standard #: 22VAC40-665-540-A Description: Based on a review of staff records on 09/10/2019, the center did not ensure that staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB) within the last 30 calendar days of the date of employment.

The tb screening in the record of Staff #5, whose date of hire was 05/28/2019, is dated 03/27/2019.Plan of Correction: Will be sure that all new hires have tb in the right time frame.
