Technical Assistance:
Inspector spoke with staff about the infant daily log and out of state child registry checks during this inspection.

Comments:
An unannounced monitoring inspection and facility tour was conducted from 10:25am-11:20am. At the time of inspectors' arrival there were 6 infants in care with 3 staff members. Children were observed participating in infant activities, interacting with staff, feeding, diapering, transitioning between activities, and nap. A sample of 5 children's records and 2 staff records were reviewed. The Staff reported that there are no medications being administered at this time. Areas of noncompliance are detailed in the violation notice and were discussed with the Director at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review and interview it was determined the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: 1. The record for child #3(enrolled 6/28/19) did not contain documentation of immunizations. Child #3 was in care during this inspection. 2. Staff confirmed that the record for child #3 did not contain immunization documentation.

Plan of Correction: Staff will request immunization documentation from parent at pick up.

Standard #: 22VAC40-185-140-A
Description: Based on record review and interview it was determined the center did not ensure that each child had a physical examination by or under the direction of a physician within the required time frame. Evidence: 1. The record for child #3(enrolled 6/28/19) did not contain documentation of a physical exam. 2. Staff confirmed there was no documentation of a physical exam in child #3's record.

Plan of Correction: Staff will request documentation of a physical exam from parents at pick up.

Standard #: 22VAC40-185-160-A
Description: Based on record review and interview it was determined the center did not ensure each staff member and individual from an independent contractor submitted documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering. Evidence: 1. The record for staff #2(hired 3/13/19) did not contain documentation of tuberculosis screening. 2. Staff #2 confirmed they did not have tuberculosis screening.

Plan of Correction: Staff #2 will get a tuberculosis screening or test.
