An unannounced monitoring inspection was completed on 09/17/2019. There were 41 children, ages 3 years-4 years, 6 staff providing direct care and 1 support staff present during the inspection. The inspector reviewed 5 children's records and the on-site staff record requirements for 6 staff members during the inspection. The staff stated that there is currently no medication being administered. Children were observed in the following activities: having breakfast, in center/free choice play, during music and movement and in large motor play outside. The inspector reviewed the emergency supplies during the inspection. Standard #: 22VAC40-185-60-A Description: Based on record review and discussion with the staff, the facility failed to ensure that the separate record for each child enrolled shall contain all of the elements as required by the standards.
Evidence: The record for Child #2 did not contain the following information: the work phone number and place of employment (employment was listed but there was not a phone number for employer provided), the name and phone number of the child's physician and the first date of attendance.Plan of Correction: The staff called the parent and got the missing information during the inspection.
