Comments:
An unannounced monitoring inspection was completed on 09/17/2019 from 9:05 A.M.-12:30 P.M. At the time of the inspection 38 children were in care with six staff and one administrators present. Six children records, six staff records, evacuations drills, first aid kits, emergency supplies, classroom observations in all classrooms, outdoor play area, and posted required information were reviewed. Children were observed during large group activities that included singing, counting, storytime, art activities, free play, small group activities that included a science experiment, listening activities, handwashing, snack time, outdoor play time, and departure. Exceptions to compliance are noted on the violation notice.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on review of children records, the center failed to obtain an immunization record before children attended the center.

Evidence:
1. Child #1's file did not contain a record of immunizations. Date of enrollment was 09/16/2019.
2. Staff verified that Child #1 did not have an immunization record on file.

Plan of Correction: Administration contacted the parent and requested a copy of immunizations be brought into the center on the child's next day of attendance at the center.

Standard #: 22VAC40-185-160-C
Description: Based on review of staff files, the center failed to ensure that one out of six staff members had an updated tuberculosis screening on file at least every two years from the date of the last screening or test.

Evidence:
1. Staff #4's last documented tuberculosis screening was dated 09/08/2017.
2. Staff verified that last documented tuberculosis screening file was dated 09/08/2017.

Plan of Correction: Administration had informed the staff member of the needed update, but the staff member had forgotten to go get the screening or test completed. The staff member will obtain the tuberculosis screening within the next two weeks.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that all hazardous materials were kept in a locked location.

Evidence:
1. In Classroom #1 there was a container of Clorox Disinfecting Wipes sitting on top of the cubbies in a basket.
2. The warning label stated "keep out of reach of children", and hazards to humans and domestic animals.
3. Staff verified that the Clorox Disinfecting Wipes were not in a locked location.

Plan of Correction: Staff immediately moved the Clorox Disinfecting Wipes to a locked location.
