Standard #: 22VAC40-185-270-A
Description: Based on observation and interview on 09/10/2019, the center did not ensure all areas and equipment of the center were maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Evidence: 1) During the inspection, the licensing inspector observed sharp and splintering wood on areas at the bottom of the playground fence on the hill towards the far right end of the playground. 2) During interview, a member of management reported several trees had been recently cleared in that area and the center is in the process of repairing the fence.

Plan of Correction: Per the Center: "Repairs to the fence began prior to the pre-license inspection and are scheduled to be fully completed by 09/20/2019, which is prior to the scheduled date the new license will take effect. Site Administration will continuously monitor building and grounds and schedule with maintenance to complete any repairs that may arise in the future."

Description: Based on observation on 09/10/2019, the center did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards.

Evidence: 1) During the inspection of the playground, the licensing inspector measured the mulch under and around the swings near the preschool building and composite climbing structure located near The Village's playground. The mulch measured approximately 3 to 5 inches in depth. The equipment requires at least six inches of resilient surfacing.

Plan of Correction: Per the Center: "Play areas were dug out and additional mulch was added to create a depth of 9 inches, which is 3 inches more than the required 6 inches. This was completed on 09/16/2019, which is prior to the scheduled date the new license will take effect. Site Administration will continuously monitor mulch depth to ensure ongoing compliance."
