Comments:
An unannounced monitoring inspection was conducted with the director beginning at 7:45 AM and ending at 9:00 AM. There were twenty-one children in the care of two staff members. The children's ages ranged between five and twelve years old. The children were observed playing games, reading, and drawing. Hand washing procedures were observed. Children were also observed eating breakfast which consisted of blueberry pancakes or cereal, dried cranberries, milk or juice. Ten children's records were reviewed. Two staff records were reviewed and in compliance. There were no medications on site to be reviewed. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The most recent emergency evacuation drill was documented as 8/29/19. The most recent shelter-in-place drill was documented as 11/26/19. The most recent fire inspection was dated 12/18/17. The most recent health inspection was dated 3/7/19.

Standard #: 22VAC40-185-130-A
Description: Based on a review of documentation, the center failed to obtain documentation of immunization for all children enrolled prior to the first day of attendance. Evidence: Child B, child C, and child D did not have documentation of immunizations in their records.

Plan of Correction: The center will request the missing documentation from the parents or guardians.

Standard #: 22VAC40-185-60-A
Description: Based on a review of documentation, not all of the children's records were complete. Evidence: Child A's record was missing the names, addresses, and phone numbers for two emergency contacts.

Plan of Correction: The center will obtain the missing documentation from the parent or guardian.

Standard #: 22VAC40-185-550-D
Description: Based on a review of documentation, the center failed to conducted monthly practice emergency evacuation drills as required. Evidence: There was no documentation of monthly evacuation drills for the month of March, April, May, and June of 2019.

Plan of Correction: The director stated that emergency evacuation drills will be conducted every month moving forward and recorded in the log.
