An unannounced Monitoring inspection was conducted on September 17, 2019. There were 15 children present, the children were observed in two classrooms. Each group was supervised by two staff members. The ages of the children ranged from 3 to 5 years.
The inspector arrived at 9:21 a.m. and departed the program at 11:45 a.m.
The children were observed during free play, outside time, and lunch.
A sample of 5 children's records was reviewed. Documentation of the fire and health inspections was reviewed. A discussion was held with the Center Lead regarding storage of staff's personal food.
Standard #: 22VAC40-185-60-A Description: Based on review of five children's record, the center failed to ensure that each child's record was complete as required by the standard.
1. Child # 5 did not have documentation of proof of birth. The child started the program on 12/5/2017.
Plan of Correction: The FSW will obtain a copy from the parents and will monitor all records for completeness.
Standard #: 22VAC40-185-270-A Description: Based on observation of the playground areas, the center failed to ensure that all areas inside and outside were maintained in a clean, safe and operable condition.
1. Observed on the playground there were weeds growing in the mulch area that had thrones. The weeds were in direct path of the play equipment. This posed a hazard to the children when playing on the equipment.
Plan of Correction: A maintenance order will be submitted, staff will follow-up to ensure work is completed.
