Comments:
Licensing Inspector arrived at 11:00 AM and departed at 12:15 PM to conduct a monitoring inspection. At the time of the inspection, there were 89 children, ages infant through preschool, in care with 14 staff. Children were observed participating in learning center activities, playing outside and eating lunch. The sample size was nine staff and nine children's records.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on inspection of the facility, it was determined that the facility did not ensure that all hazardous substances were kept in a locked place.

Evidence:
There was air freshener stored in an unlocked cabinet in the younger toddler room. Additionally, the cleaner for the diaper changing table was stored on the diaper changing table.

Plan of Correction: We will ensure that any hazardous substances not in use will be put away in a locked cabinet or closet.

Description: Based on observation, it was determined that the facility did not ensure that all requirements related to hand washing were followed.


Evidence:
1. Children:
a. Children in the two toddler classrooms did not have their hands washed after their diaper changes.
b. Children in the two year old classroom did not wash their hands after lunch.
2. Staff:
a. Staff member in the older toddler room did not wash their hands, or change gloves between changing children's diapers.

Plan of Correction: Staff were provided the proper hand washing requirements from the Standards for Licensed Child Day Centers. Reviews / demonstrations will also be held during department meetings.

Description: Based on observation, it was determined that the facility did not ensure that all requirements related to diapering were maintained.

Evidence:
1. In the younger toddler room, staff member lifted the lid of the foot operated diaper disposal system with their hand to dispose of diapers.
2. In the older toddler room, staff member was observed disposing of diapers in a plastic bag hanging on the diaper changing table rather than the foot operated disposal system.

Plan of Correction: Staff were provided the proper diapering requirements from the Standards for Licensed Child Day Centers. Reviews / demonstrations will also be held during department meetings.

Standard #: 22VAC40-185-510-N
Description: Based on review of medication at the facility, it was determined that the facility did not ensure that medication was returned to parents within 14 days of the expiration of the authorization.

Evidence:
There was a bottle of Children's Motrin that had been authorized for only seven days in July that had not been returned to the parent.

Plan of Correction: The medication was sent home with the parent the afternoon of 9/11/19.
