Technical Assistance:
Discussed the Emergency Action Plan with the Director.

Comments:
An unannounced renewal inspection was completed today. The physical plant, 4 staff files, 5 children's files, annual fire inspection, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected between 11:30am-12:30pm. There were 9 children under the supervision of 2 staff. During the inspection, the children were observed eating lunch, cleaning up, reading books and enjoying free play. Each day the children are in care, they bring their lunches from home. The interactions between the children and staff were seen to be positive and constructive in nature. Description: Based on file review, the center did not include all required information on an Injury-Accident Report completed for a child in care.
Evidence
1. On 5/17/19, a child "fell and skinned" her knees. This report does not include either a staff and parent signature or two staff signatures.

Plan of Correction: This information will be added to reports when completed, effective immediately.
