63.2(17) License & Registration Procedures

Comments:
A mandated Renewal inspection was conducted today from 10:30 am to 2:00 pm. There were 51 children present with 7 staff. Children were observed practicing their letters on chalkboards, sounding out words, having lunch, and playing outside on the playground. Reviewed 4 children's files, 5 staff files, medications and authorizations, fire and health inspections, emergency evacuation log, liability insurance, and physical plant. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not have a work phone number and place of employment of each parent for each child enrolled.
Evidence:
Child#1 and Child#2 were missing both parents' work number and place of employment.

Plan of Correction: An email was sent to the parents requesting the information.

Standard #: 22VAC40-185-70-A
Description: Based on record review, documentation that two or more references as to character and reputation as well as competency were not checked before employment or volunteering.
Evidence:
Staff#5 (start date 11/1/18) did not have documentation that 2 or more references were checked prior to employment.

Plan of Correction: I will re-call the references and document.

Standard #: 22VAC40-185-510-E
Description: Based on observation, medications were not be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.
Evidence:
Child #5's medication (children's zyrtec) was not labeled with child's information.

Plan of Correction: This was corrected at time of visit by labeling with all child's information, and dosage amount and time.

Standard #: 22VAC40-185-560-G
Description: Based on observation, food brought from home was not labeled with child's name and date,
Evidence:
In Preschool Room #2, a lunch bag brought from home with food to be consumed today had a date of 9/16/19.

Plan of Correction: This was corrected at time of inspection by writing today's date on the tag.

Standard #: 63.2(17)-1721.1-B-2
Description: Based on record review, a criminal history result was not obtained for Center Staff or Agents.
Evidence:
Staff#6 did not have documentation of a criminal history result available for review.

Plan of Correction: We will have the fingerprints re-done as soon possible.
