Technical Assistance:
Discussed with the Director that gates on the playground should not pose a head entrapment hazard. A head entrapment hazard is a gap in the fence which allows a 3-1/2 inch by 6-1/4 inch rectangle to fit through but does not allow a nine-inch circle to fit through. Also discussed with the Director the need to document attempts to obtain out of state background checks and to ensure all staff are documenting the times that parents are notified of injuries on accident/incident reports.

Comments:
An unannounced monitoring inspection was conducted today with the Director. There were 66 children in care (ages five months to 4-years-old) supervised by 12 staff. Children were observed eating lunch, washing hands, preparing for nap, and napping. Six staff files and five children's files were reviewed and were in compliance. The playground was inspected. The emergency drill log was reviewed and was in compliance. The accident/incident log was reviewed. Three medications and paperwork were reviewed and were in compliance. There are seven staff currently certified in MAT (Medication Administration Training).

The Center self-reported a concern involving rough handling of a child by a staff member. Interviews were conducted and written documentation was reviewed. There were no violations as a result of this inspection.

Description: Based on observation not all areas of the center inside and outside were maintained in a safe condition.

Evidence: 1. On the Toddlers playground the top rung of the fence was separating from the lattice. A pole had pulled away from the fence that poses an impalement hazard. 2. In the Prekindergarten 2 and Intermediates classrooms the phone cords were hanging low enough to be within reach of children and are strangulation hazards.

Plan of Correction: I am going to call our Maintenance person today and see when he can come out and fix the fencing/gates.

I will remove the phone cords today (9/13/19) and will purchase shorter phone cords that should be received by 9/18/19.
