Comments:
The licensing inspector conducted an unannounced inspection on Tuesday, August 20, 2019 in response to a self-reported incident that the Department received on August 12, 2019 regarding supervision. The inspector was on site at the center on August 20, 2019 from 9:00am to 3:40pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed.

The preponderance of the evidence gathered during the investigation validates the self-reported incident regarding supervision. A violation was cited regarding supervision. See the violation notice for more details.

Violations:
Standard #: 22VAC40-185-340-A
Description: Based on interviews conducted on 08/20/2019, the center did not provide adequate care and protection for an infant in care that received another child's breast milk.
Evidence:
1. The center's representative, staff A and staff B reported on 08/12/2019, staff A fed child A (3-month-old) less than one ounce of breast milk belonging to child B (3-month-old).

2. An interview with staff A revealed that on 8/12/2019, staff A was holding child A. Staff B gave staff A a warmed bottle containing breast milk belonging to child B. Staff A stated, "I guess because I was holding [child A], I thought [staff B] said [child A]." The bottle had a red label with child B's name on it.
3. An interview with staff B revealed that on 8/12/2019, staff B gave staff A child B's bottle and stated " this is [child B's] bottle." Staff B also stated that when they noticed child A was being fed the wrong bottle they ran to staff A and took the bottle out of child A's mouth and child A drank less than an ounce.
4. The center's safe bottle feeding posting instructs teachers to do the following before feeding a child, "Before feeding the child, match the label on the bottle to the child's face. Confer with your co-teacher that you have the correct bottle and the correct child." Staff A did not follow the centers safe bottle feeding posting.

Plan of Correction: Management will complete retraining with staff on bottle feeding best practices and name to face verification. Any new incoming staff or current staff filling in will go over the Positive Guidance standards which includes our name to face bottle verification one pager. Management will use Professional Development Day to go over again the policies around safe bottle feeding. Observations will be completed in the classroom to ensure that our positive guidance practices are in place.
