The licensing inspector conducted an unannounced inspection on Monday, September 16, 2019 in response to a self-reported incident that the Department received on August 27, 2019 regarding administration, supervision, first aid supplies and injury reporting. The inspector was on site at the center on Monday, September 16, 2019 from 02:25pm to 3:54pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed.

The preponderance of the evidence gathered during the investigation did not support violations of the standards.Though not a part of the original report, there were violations found during the inspection regarding staff records. See the violation notice for more details.

Standard #: 22VAC40-185-160-C
Description: Based on record review, the center did not ensure that at least every two years from the date of the first initial screening or testing, or more frequently as needed if recommended by a licensed physician or the local health department, staff members obtain and submit the results of a follow-up tuberculosis screening.
The record of staff #1 (start date: 12/19/07) did not have documentation of an up-to-date tuberculosis screening. Staff #1 had documentation of a tuberculosis screening dated 7/12/17.

Plan of Correction: We will ensure that staff #2's TB is updated within one week.
