Violations:
Standard #: 22VAC40-665-520-B-11
Description: Based on review, the center did not obtain a written allergy care plan for each child with a diagnosed food allergy to include instructions from a physician regarding the food allergy to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
1. Six children (Child #1 thru Child #6) were listed to have a food allergy at the center and the center did not have a food allergy care plan in the records.

Plan of Correction: I will request a allergy care plan from the parents.
