Comments:
A Subsidy Health and Safety inspection was conducted today from 7:15am to 8:10am to review supplemental health and safety requirements. A total of 37 children were present with 5 caregivers. Children were observed playing board games, participating in a staff led activity learning American Sign Language and engaging in group exercise. Programming, menus, emergency procedures, 2 medications, allergy care plans, and 6 children's records and 3 staff records were reviewed.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on review of children's records and staff interview, it was determined that not all children with a diagnosed food allergy had documentation of a written allergy care plan on file.

Evidence:
1. Child #1 has a diagnosed food allergy and did not have a written allergy care plan on file.

Plan of Correction: I'm going to ask the mother if she has a new physical. That physical was when the child was in 2nd grade and the child is now in 5th grade. If the child still has the food allergy I will give the parents the necessary paperwork to get an allergy care plan signed by a physician.
