An unannounced renewal inspection was conducted on 9/20/19 from 11:55am until 2:20pm. At the time of the tour, there were 25 children in care with 4 staff present. A sample of 5 children's records and 5 staff records were reviewed. Children were observed resting quietly during nap and participating in story time. The emergency preparedness plan, first aid and emergency supplies, documentation of emergency practice drills, children's injury reports and center postings were reviewed. There are currently no children in care that require medication administration. Two violations in the areas of administration and background checks are identified on the violation notice and were discussed with the center owner during the exit interview.

Standard #: 22VAC40-185-140-A Description: Based on record review and interview, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the chid's attendance or within one month after attendance.
Evidence:
1. The record for child 1 does not contain documentation of a physical examination. Child 1 has a date of enrollment of 6/3/19 and was in care on the day of the insepction.
2. Staff 1 and staff 2 confirmed that the record for child 1 was lacking documentation of a physical examination.Plan of Correction: The parent of child 1 was contacted during the inspection and will bring a copy of the child's most recent physical this afternoon at pick-up.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on record review and interview, it was determined that the licensee did not ensure that all employees hired prior to January 22, 2018 have a completed fingerprint based national criminal history record check by September 30, 2018.
Evidence:
1. The record for staff 1 does not contain documentation of a completed fingerprint based national criminal history record check. There was a completed Virginia criminal record check dated 9/17/15.
2. Staff 1 confirmed that she had not completed a fingerprint based background check by September 30, 2018.Plan of Correction: Staff 1 scheduled and completed an appointment for the fingerprint check on 9/10/19. The results will be placed in staff 1's record and forwarded to the licensing inspector once received by the center.
