Comments:
A subsidy health and safety inspection was conducted today with the interim director, who was present with three staff and 38 children. The center is up to date with their lock-down drill. Three allergy plans were reviewed today.
Arrival time: 4:30 pm
Departure time: 5:40pm

Violations:
Standard #: 22VAC40-665-520-B-11
Description: Based on a review of the allergy list and an interview with the director, it was determined that three children with diagnosed food allergies did not have an allergy plan on site.
Evidence:
Child A has a shellfish allergy; child B has a fish allergy and child C has an egg allergy and none of these children have an allergy plan on site.

Plan of Correction: We have already contacted each parent and we are waiting for them to bring in the allergy plans from their physician.
