Comments:
An unannounced monitoring inspection was conducted on September 11, 2019 from approximately 9:00 am to 11:50 am. Nine staff were present with forty-seven children. Additional staff were present for administrative and kitchen duties. Five children and five staff records were reviewed.

The providers responses for the ?Description of Action To Be Taken? were not received as of September 24, 2019 and will not appear on this ?Violation Notice?.

Standard #: 22VAC40-185-140-A
Description: Based on review, facility did not ensure 1 of 5 children's records has a physical examination within a month of beginning attendance.

Evidence: The record for child #5 (start date 8-8-19) did not have a physical.

Description: Based on review, facility did not ensure 3 of 5 children's records have a physical examination and immunization record that is signed by a physician, designee or official of a local health department.

Evidence: The following was found during record review:
1. The record for child #2 (start date 3-5-18) had a physical and immunization record that were not signed.
2. The record for child #4 (start date 8-24-16) had a physical that was not signed.
3. The record for child #5 (start date 8-8-19) had an immunization record that was not signed.

Description: Based on review, facility did not ensure 3 of 5 children's records do not have documentation of required information.

Evidence: The following was found during record review:
1. The record for child #2 (start date 3-5-2018) did not have father's place of employment and father's employment phone number.
2. The record for child #3 (start date 4-3-2017) did not have the address for the second emergency contact.
3. The record for child #5 (start date 8-8-2019) did not have father's home information and father's employment information.

Evidence: The following was found during record review:
1. The record for staff #2 had documentation of completing 5.5 hours of training in 2018 however staff report at least 16 hours of training were completed.
2. The record for staff #3 had no documentation of completing training in 2018 however staff report at least 16 hours of training were completed.
3. The record for staff #4 had documentation of completing 5.5 hours of training in 2018 however staff report at least 16 hours of training were completed.
4. The record for staff #5 (start date 6-3-2019) did not have documentation of emergency contact address.

Description: Based on observation, facility did not ensure areas and equipment of the center are maintained in a clean, safe and operable condition.

Evidence: In the preschool room, there was a 4" x 5" square speaker on top of the cabinet. The cord was hanging down within 3' of the floor within reach of children.

Description: Based on observation, facility did not ensure hazardous substances are kept in a locked place.

Evidence: Multiple cleaners labeled with "warnings/caution...keep out of reach of children" were observed in an unlocked storage closet in the center of the facility and in an unlocked cabinet in the toddler classroom.
