Technical Assistance:
1. Reviewed attendance record requirements. 2. Reviewed/trained vendor on new subsidy requirements. 3. Model forms provided: written permission for emergency medical care, emergency preparedness plan, and swim permission.

Comments:
10:10am-12:00pm Conducted an announced subsidy inspection at he vendor's new address. Observed 1 child = 2 points with the vendor. Ratios and supervision are in compliance. The child was eating a morning snack, playing in the pretend kitchen, and playing with play dough. The home was found to be very clean, safeguarded appropriately, and sufficiently supplied with toys and equipment for the children. Recordkeeping and emergency supply non-compliances found.

Violations:
Standard #: 22VAC40-665-170-B
Description: Based on records reviewed, it was determined that 2 of 2 children's records did not include required information. Evidence: Child #1 and #2 - Name, address and phone number of an emergency contact person; and written permission for emergency medical care.

Plan of Correction: The vendor will ask the parent to provide this information.

Standard #: 22VAC40-665-190-B
Description: Based on records reviewed, it was determined that the vendor has not had a TB screening in the last 2 years ago. Evidence: The vendor's previous TB test expired in March 2019.

Plan of Correction: The vendor made an appointment to get a TB test for tomorrow.

Standard #: 22VAC40-665-390-B-2
Description: Based on observations made, it was determined that there was not a working battery-operated radio. Evidence: The batteries were dead in the radio.

Plan of Correction: The vendor will buy new batteries for the radio.
