Comments:
A Subsidy Health and Safety inspection was conducted on on 9/19/19 from 10:20am - 12:15pm. During the inspection there were 18 children ages one year old through five years old in care with five staff. Children were observed participating in various activities in the classrooms, playing outside, eating lunch, and during rest period. Records were reviewed for five children and five staff. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements, were discussed during the exit interview, and are identified on the violation notice.

Description: Based on a review of the center's policies and interviews, it was determined that the vendor did not ensure that they shall develop and implement a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members.

Evidence:
1. There was no written policy and procedure regarding consistent staff in each group of children available for viewing during the inspection.
2. Staff #1 confirmed that a written policy and procedure regarding consistent staff in each group of children had been developed.

Plan of Correction: The facility responded: A written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members will be completed and implemented.
