Comments:
A Subsidy Health and Safety inspection was conducted on on 3/18/19 from 8:45am - 10:15am. During the inspection there were 5 children ages one year old through three years old in care with three staff. Children were observed participating in various activities in the classrooms and eating morning snack. Records were reviewed for five children and five staff. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements, were discussed during the exit interview, and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-510-B
Description: Based on observation and interviews, it was determined that the vendor did not ensure that for each group of children, the vendor shall maintain a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence:
1. The attendance record for multiple classrooms did not have the the time of arrival for all children present.
2. Staff #6 confirmed that the arrival of all children was not documented as the attendance in the room just indicates whether the child was present and some parents did not sign their children in at the front desk.

Plan of Correction: The facility responded: Going forward we will record the time of arrival and the time of departure on the attendance sheet located in each classroom.

Standard #: 22VAC40-665-580-E-1
Description: Based a review of five staff records, it was determined that the vendor did not ensure that all staff have a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care.

Evidence:
1. The record for staff #1 (date of hire (3/16/15) did not contain documentation for a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. There was a CPR certification card, however the certification expired in June 2019.
2. The record for staff #3 (date of hire (1/21/18) did not contain documentation for a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. There was a CPR certification card, however the certification expired in June 2019.
3. The record for staff #4 (date of hire (9/30/16) did not contain documentation for a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. There was a CPR certification card, however the certification expired in June 2019.
4. Administrative confirmed there was not documentation for to demonstrate that staff #1, staff #3 and staff #4 had a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care.

Plan of Correction: The vendor responded: All staff will complete a course to obtain certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. All new staff will complete a course to obtain certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care within 30 days of employment
